Mammograms: Screening for Breast Health | Fullerton, CA

What Are Mammograms?

Mammograms involve taking X-ray images of the breasts to identify abnormal changes, such as lumps or calcifications, that may indicate cancerous or precancerous conditions. The procedure is typically performed by a radiologic technologist and reviewed by a radiologist specializing in breast imaging.

Recommended Screening Age

For women with an average risk of breast cancer, guidelines recommend starting regular mammogram screenings at around age 40. Screening intervals are generally recommended every 1-2 years, depending on individual risk factors and medical history. However, some organizations, such as the American Cancer Society, suggest that women may choose to start screening at age 45 and switch to annual screening at age 55.

Factors Indicating Earlier Screening

Several factors may warrant earlier mammogram screening:

  1. Family History: Women with a strong family history of breast or ovarian cancer may be advised to begin mammograms earlier than age 40, typically around 10 years earlier than the age at which the youngest relative was diagnosed.
  2. Genetic Mutations: Presence of certain genetic mutations, such as BRCA1 or BRCA2, significantly increases the risk of breast cancer. Women with these mutations may need to start screening in their 20s or 30s, depending on individual risk assessments.
  3. Personal History: Women who have previously had breast cancer or other breast conditions that increase cancer risk may require earlier or more frequent mammogram screenings.
  4. Radiation Exposure: Previous radiation therapy to the chest area, especially during childhood or adolescence, may increase the risk of developing breast cancer and necessitate earlier screening.

Mammograms are essential tools for detecting breast cancer early, when treatment is most effective. Understanding when to start screening and considering individual risk factors are crucial for making informed decisions about breast health. Regular discussions with healthcare providers can help determine the most appropriate screening schedule based on personal risk factors and medical history. Early detection through mammograms can potentially save lives by facilitating timely treatment and improving outcomes for breast cancer patients.

Breast cancer remains a prevalent concern, affecting nearly 300,000 individuals annually, irrespective of age or gender. Detecting potential signs early can significantly impact outcomes, making awareness and regular screening essential.

At-Home Screening Tips:

Perform a self-examination monthly, ideally around the time of your menstrual cycle:

  • Raise one arm and relax it.
  • Use your opposite hand with two fingers to gently explore your breast in a spiral pattern, feeling for any abnormalities such as:
    • Nipple tenderness, lumps, or thickening in or around the breast or underarm area.
    • Changes in skin texture or enlarged pores.
    • Any new or unusual lump, regardless of size.

Pay Attention to Changes:

Additionally, monitor your nipples for:

  • Unexplained swelling or shrinkage.
  • Recent asymmetry or changes in size or shape.
  • Dimpling or puckering of the skin.
  • Inward or inverted nipples.
  • Red, scaly, swollen skin resembling an orange peel texture.
  • Nipple discharge.

Taking Action:

Regular at-home exams are crucial for early detection. If you notice any concerning symptoms, such as lumps, tenderness, or visual changes in your breast, consult your doctor promptly for evaluation.

Breast Cancer Awareness: Checking Your Breasts | Fullerton, CA

Breast cancer awareness isn’t something that should only be once a year because it is something that affects us year-round. And while we love to wear our pink stuff, making sure you are healthy is the best kind of support you can give. We can do this by regularly checking our breasts in between yearly mammogram appointments. Here is a quick breakdown:

Step 1. Begin by looking at your breasts in the mirror with your shoulders straight and your arms on your hips, looking for unusual size, shape, and color, visible distortion or swelling. Make sure to schedule a doctor’s appointment if you notice anything odd.

Step 2. Raise your arms and look for the same changes.

Step 3. While you’re at the mirror, look for any signs of fluid coming out of one or both nipples (this could be a watery, milky, or yellow fluid or blood).

Step 4. Feel your breasts while lying down, using your right hand to feel your left breast and then your left hand to feel your right breast. Use a firm, smooth touch with using your finger pads, keeping them flat and together. Use a circular motion, about the size of a quarter.

Cover the entire breast from top to bottom, side to side — from your collarbone to the top of your abdomen, and from your armpit to your cleavage.

Step 5. Finally, feel your breasts while you are standing or sitting. Many women find that the easiest way to feel their breasts is when their skin is wet and slippery, so they like to do this step in the shower.

If you find that you may be feeling something strange in any of these areas, make an appointment for a medical exam.

Good News Regarding Hormone Replacement Therapy | Fullerton, CA

When it comes to menopause, there has been a lot of controversy surrounding the involvement of hormone therapy. Traditionally, doctors would provide hormone replacement therapy (HRT) as a treatment option in order to augment the patient’s hormone levels. However, in 2002, the Women’s Health Initiative, a government-funded study took place and revealed many questions regarding the HRT’s benefits and risks.

In 2012, The North American Menopause Society released a statement stating, “the concept of lowest dose for the shortest period of time was inadequate or even harmful for some women”. But in 2017 the stance had changed. They released a paper stating that the use of hormone replacement therapy for menopausal and postmenopausal women “guarantees to make healthcare providers and the women they treat more comfortable using HRT when women want it to improve their quality of life”.

So, what does that mean for women going forward? Here are some of the highlights:

  • The benefit-risk ratio is most favorable for women in their 50’s with hot flashes and night sweats, or at elevated risk for bone loss or fracture. Those younger may not have as many advantages.
  • Longer durations of HRT should be for documented indications and discussed with a physician.
  • Women with vulvar and vaginal symptoms, or with a loss of libido, can benefit from having hormone replacement therapy for sexual-function difficulties.
  • Women who enter menopause early will experience an estrogen-deficiency related consequence. Early estrogen treatment, in addition to endometrial protection.

Is Hormone Therapy Bad for Me? | Fullerton, CA

The history of hormone replacement in women has seen many swings in popularity over the past few decades. In 1991, the National Institutes of Health (N.I.H.) launched the Women’s Health Initiative (WHI), designed to test the effects of hormone therapy on heart disease, bone fractures, breast cancer and colon cancer. One group of women who had undergone a hysterectomy received oral estrogen (Premarin) alone [estrogen replacement therapy or ERT]. A second group received a combination of Premarin and progesterone (PremPro) [combination hormone replacement therapy or HRT]. A third group received a placebo, which contained no hormone. In 2002, the study was stopped because the Provera group was associated with a slight increase in the incidence of breast cancer, heart attack, stroke and blood clots.

In April 2011, the Journal of the American Medical Association (JAMA) published a follow-up study of the women in the WHI who took estrogen alone. The finding: a statistically significant reduction in breast cancer over time. Among the women with a prior hysterectomy treated with estrogen alone and followed for 11 years, there was NO increased risk of heart attack, stroke, blood clots, hip fracture, colon cancer or mortality.

After a decade of fear and confusion regarding the risks and benefits of estrogen replacement therapy, medical studies have now clearly established its safety and its health benefits. In March 2012, the North American Menopause Society released a statement that supports these findings, stating that combination hormone therapy (both estrogen and progesterone) initiated around the time of menopause is safe.
